Effect on circulation of conversion of atrial fibrillation to sinus rhythm.

چکیده
U NTIL recently enthusiasm for conversion of atrial fibrillation to sinus rhythm in patients with rheumatic heart disease has been dampened by the high rate of reversion to atrial fibrillation soon after conversion. Mitral commissurotomy, if followed by a reduction of left atrial pressure and size, offers the theoretical possibility of a reduction in this reversion rate. Successful commissurotomy of itself, however, does not restore sinus rhythm, and many patients previously in sinus rhythm will fibrillate following surgery. Conversion with quinidine is not a benign procedure and sudden death during conversion attempts has been reported.1Thus the increasing number of patients presenting as candidates for conversion has increased the need for a better understanding of the alterations in cardiac performance that occur as a result of atrial fibrillation and the changes to be expected upon conversion. In 1924, Blumgart2 demonstrated a reduction in heart rate during exercise following conversion to sinus rhythm. Since then, several additional reports have been published comparing various aspects of cardiac performance during exercise before and after conversion from atrial fibrillation to sinus rhythm.3-7 Nevertheless, many aspects of the problem are still in doubt for the following reasons: duplicate control studies in subjects receiving quinidine but not converting have been lacking; it is difficult to determine in most cases how adequately digitalized were

Intravenous Methylprednisolone, a Possible Cause of the Atrial Fibrillation
We are presenting a case illustrating atrial fibrillation (AF) following the use of methylprednisolone in a patient with pelvic and femur fracture. A 48- year- old man with no significant past medical history, was admitted to the emergency department after injury in a car accident. He suffered a multiple bone fracture with chief complaints of pain and shortness of breath. He was transfer...
متن کاملEffect of atrial fibrillation on cardiac output.
The introduction by Lown, Amarasingham, and Neuman (1962) of a safe, simple, and effective method of converting atrial fibrillation to sinus rhythm has increased the interest in the effects of this arrhythmia on the circulation. Previous reports indicate scant agreement regarding the effects of atrial fibrillation on the cardiac output. Studies on groups of patients with and without atrial fibr...
متن کاملInvestigating the Effect of Inflammation on Atrial Fibrillation Occurrence by Measuring Highly Sensitive C-reactive Protein (hs-CRP)
Introduction: Atrial fibrillation (AF) is the most prevalent cardiac arrhythmias that cardiologists and internists encounter. The goal of this article is to clarify an overview of the evidence linking inflammation to AF existence, which may highlight the effect of some pharmacological agents that have genuine potential to reduce the clinical burden of AF by modulating inflammatory pathways. Mat...
متن کاملLeft ventricular ejection time in atrial fibrillation.
Left ventricular ejection time (LVET; indirect carotid pulse) was studied in 48 patients with atrial fibrillation. In comparison with normal individuals in normal sinus rhythm, ejection times for similar cycle lengths were usually short in patients with atrial fibrillation. Patients without mitral valve disease usually show increasing ejection times following increasing cycle lengths until a ce...
متن کاملAlterations in cardiac function immediately following electrical conversion of atrial fibrillation to normal sinus rhythm.
SUMMARY An assessment was made of cardiac function by means of right heart catheterization before and immediately after conversion of atrial fibrillation to sinus rhythm in 11 studies on 10 human subjects. Comparable data during exercise were available in five studies. Medical management of patients in this study was identical to that employed routinely for this procedure. After conversion to n...
